This document provides step-by-step guidance for families, early intervention services, schools and community programs on the subject of early childhood transition. Topics addressed include the initial planning meeting, visiting program options, SoonerStart evaluation and assessment, IFSP review, the transition planning conference, school district determination of eligibility, and the IEP meeting. Although parts of the document are state-specific, the document could easily be modified for use in other states.
