This resource brings together strategies, practices, resources, and documents on transition to enable administrators and service providers to deliver high-quality transition services for children and youth moving into, through, and out of education programs within the juvenile justice system.  The focus is on the administrative processes, coordination efforts, and communication practices within the juvenile justice system -- with a primary focus on programs that directly provide education services. This document provides step-by-step guidance for families, early intervention services, schools and community programs on the subject of early childhood transition. Topics addressed include the initial planning meeting, visiting program options, SoonerStart evaluation and assessment, IFSP review, the transition planning conference, school district determination of eligibility, and the IEP meeting. Although parts of the document are state-specific, the document could easily be modified for use in other states.

The second annual Pennsylvania Early Childhood Conference in 2006 featured information on transition. Presenters from the University of Virginia's Curry School of Education provided valuable insights and strategies to enhance the transition process for children and families.
